CONCIERGE SERVICES MEMBER AGREEMENT

Stephen Witte agrees that a Concierge Services Member will receive:

>> Advisory services, in response to issues raised by Members
>> Consulting services, in providing ideas and information proactively to Members
>> Advance information and findings for Members from ongoing research projects
>> Individualized response to each Member request, with prompt and confidential service
>> Same day or next-day appointments for the Member or Member’s staff
>> Appointments by phone or in-person
>> In-person or on-site appointments in the U.S. or worldwide
>> Direct contact via a dedicated mobile phone for exclusive use of Concierge Members
>> Assurance of no more than a total of six (6) Memberships per quarter
>> Member identities will remain confidential during Membership and thereafter

The Concierge Services Member accepts these benefits and the following terms and conditions:

>> Remit a Quarterly Membership Deposit of $5,400, due with a signed Agreement
>> Membership is offered by quarter (3 months) and can be terminated with 30 days notice
>> Members who terminate the Agreement will receive a prorated refund of remaining account balance
>> Member Agreement cannot be transferred without written consent of Member and Stephen Witte
>> Stephen Witte may terminate an Agreement with thirty days (30) written notice and prorated refund
>> Standard rate of $300/hr will be deducted from the account balance; additional hours at $300/hr
>> Members have the option of using their account balance toward Agency Services
>> Members receive a 20% savings on Programs and Training by Stephen Witte Associates
>> Transit rate is $150/hr deducted from the account balance for travel time
>> Expenses for domestic and/or international travel will be billed to the Member
>> Airlines will be used unless Member’s scheduling requires a private aircraft charter
>> Amenities covered by the Member Agreement are limited to Concierge benefits as described
>> Amenities and services excluded from the Member Agreement to be paid separately by the Member
>> For access, Members receive a dedicated mobile number, to be changed each quarter
>> Stephen Witte answers these calls (no administrative support) or will call back soonest
>> Concierge Services are available during holidays on a call-back basis
>> Member Agreements will be governed by and construed in accordance with Indiana state law.
